Set Up Your Survey
Survey Title
Enter a clear title for your survey. This is how the survey will appear to admins and employees.
Landing Page
Select the landing page for the survey. Submit New Survey is selected by default.
Survey Description
Add a short description of up to 80 characters to explain the purpose of the survey.
Display Order
Set the display order for the survey in the My Workspace program list.
Choose a Survey Template
Select a Template
Choose from a library of ready-to-use templates organized by category. Templates can include predefined questions, themes, benchmarks, and recommended setup.
Create Your Own Survey
Select Custom or Start From Scratch to build your own survey.
Form Builder
Use the Form Builder to edit a selected template or create a new survey. Organize your survey into Themes and Questions.
Configure Survey Insights
Most Engaged Users
Choose the term you want to use for your most engaged users. For example, Best Performers.
Minimum Response Threshold
Set the minimum number of responses required before survey results and insights are displayed. This prevents managers or admins from drawing conclusions from very small groups.
Configure Survey Availability
Survey Open Date and Time
Set the date and time when the survey becomes available to respondents.
Anonymize Responses
Choose whether survey responses should be anonymized.
Include Inactive Users
Choose whether inactive users in the program should be included as respondents.
Restrict Responses
Enable this option if each respondent should be allowed to submit the survey only once.
Customize the Survey Experience
Welcome Message
Add a welcome message of up to 300 characters. For example: Your input matters!
Program Tile Image
Add an image to bring your survey program tile to life. Images should not exceed 500 KB.
Invite Email
A standard survey invitation email is provided. Customize the message to match your organization’s communication style.
Invite SMS
A standard SMS invitation is provided. Customize the message as needed.
Access Rules
Define who can access the survey by combining user core attributes. Use access rules to target specific groups, teams, locations, or other employee attributes.
Set Up Reminders and Action Plans
Configure a workflow to automatically send reminders to non-respondents at a defined frequency while the survey is open. Before publishing, review the available benchmarks and action plans to ensure the survey has the right comparison data and recommended follow-up actions enabled.
Restart a Survey
For recurring surveys, admins can restart the survey to invite all eligible users to respond again as if it were a new survey. Previous responses remain intact and are not overwritten. This makes it easy to run monthly, quarterly, or annual pulse surveys and track how scores for themes and questions are progressing or declining MoM, QoQ, or YoY, helping managers identify trends and take timely corrective action.

Publish the Survey
Review the survey settings, questions, access rules, invitation messages, and availability before publishing. Once published, the survey becomes available to the users defined by your access rules.
